When a Vendor or Attorney cannot access the DocuSign Form

The Vendor or Attorney does not need to provide a password to access our DocuSign forms. They might have had a profile already created with DocuSign which is why they are getting the password message to log in. 


If the Attorney or Vendor has used DocuSign before and they had created a profile/account in the past, in their name/company or spouse name,  they should log out and open our DOCUSIGN FORM.  If that does not work, the vendor or attorney should go into another new browser to open the link from the Comptroller. If they need help on how to fill the forms, there is a job aid for for EFT/W9 for vendors only.
